ARA-290

ARA-290 (cibinetide) is a synthetic 11-amino-acid peptide derived from the helix-B surface region of erythropoietin (EPO). It was deliberately engineered to strip out EPO's erythropoietic activity, the stimulation of red blood cell production, while retaining its separate tissue-protective signaling function.

The mechanistic basis for this separation lies in receptor selectivity. Native EPO acts through the classical homodimeric erythropoietin receptor (EPOR) to drive erythropoiesis. ARA-290 instead selectively activates the innate repair receptor (IRR), a distinct heteromeric complex formed by EPOR and the beta-common receptor (CD131/βcR). This receptor complex is notably upregulated specifically in injured or inflamed tissue and largely absent in healthy cells, meaning ARA-290's activity is concentrated where tissue damage signaling is already present. IRR activation initiates anti-apoptotic signaling and suppresses local pro-inflammatory cytokine cascades, without engaging the hematopoietic pathway responsible for EPO's cardiovascular risk profile. ARA-290 is studied in laboratory research as a tool for isolating EPO's non-hematopoietic, tissue-protective signaling from its erythropoietic effects.
